ANSWER: Improperly wired outlet and light fixture.

This is an actual picture, taken at an actual U.S. Inspect inspection. What's wrong with this picture? Well this one is pretty obvious. It is some homeowner-installed improper wiring. This is a true electrical hazard that would require removal of the light fixture and rewiring of the outlet with final installation of a protective outlet cover.
